So my current preventatives are starting to quit on me. I was not pain free but they were working better than anything else I had previously tried. I am currently taking 200mg topamax, 200mg lyrica and 160mg verapimil. I have also been prescribed various narcotics to deal with migraine attacks that work OK though I do still end up in the ER about once every 2 months or so. My neurologist has me increasing my topamax to 250mg and I see a pain specialist every 2 weeks for cortisone injections in the base of my neck.

I've tried botox, neurontin, amitriptyline, I can't take beta blockers or triptans, I have so so luck with cafergot as an abortive.

The cortisone injections are really really painful and I get maybe 3 or 4 days of mild relief and all of that other stuff didn't work. What else has worked for people? I am currently at a 5x/week migraineur and it sort of sucks!

I think the problem with preventatives is that different preventatives work for different people and they don't know if a preventative is going to work for you until you give it a go for a good couple of months. It's a hit or miss game. I've had migraines for over 20 years and have tried nearly every preventative. Topomax worked for me but lost its effectiveness. Now I am on zonegran. I get CDH/migraine. They are less severe but I am still medicated 5-7 days a week. I wish you luck in finding relief and pain free days for everyone.
